To maximize your insurance coverage for your dental needs, we will help you to interpret your insurance benefits if you kindly bring in a copy of your insurance benefits plan.  Know that your insurance company will not guarantee benefits; nor do we.  Please be aware that your plan may have special clauses indicating reduction in benefit due to eligibility, a change in treatment, modified plan benefits, co-ordination of benefits, waiting period, missing tooth clause, downgraded fillings for white vs. silver or downgraded crowns, frequency limits or annual maximums; in which we cannot interpret for you.

Annual Fees:

Adults:  $425.00/year   Value $827.00

Children: $375.00/year (15 and under)  Value:  $676.00

 

Our In-House Discount Dental Plan Will Provide You With:

  • 2 Cleanings per year (a value of $240.00)
  • 2 Exams per year (value of $187.00)
  • 1 Emergency Exam per year (value of $107.00)
  • 1 Emergency X-ray per year (value of $48.00)
  • Bite-wing X-rays every 18 months (value of $85.00)
  • Panorex X-ray every 5 years (value of $160.00)
  • One fluoride application per year (under 16 years old:  with a value of $59.00)

In addition to the above services, patients will receive a 20% discount on all other dental service with payment at time of service with cash or check.

Terms and Limitations of the Plan:

  • Plans purchased are for that individual only; they cannot be shared and are non-transferable.
  • This plan is good for one year from date of purchase and no benefits will be provided after the termination date.
  • Cleanings must be performed before the end of coverage date.
  • There will be additional charges if Gum Disease is present.
  • This is a dental discount plan.  It is NOT dental insurance.  It cannot be combined with any other dental insurance, promotions, or coupons.
  • To receive the discount payment must be paid in full at the time of service for any work received.
  • Annual fees for the discount dental plan are subject to change in the future.
  • Membership fees are non-refundable.
  • Children’s fees apply those who are under the age of 16 years old at the time of purchase.
  • Membership is good only at Galleria West Family Dental.  Therefore, if you are referred to a specialist, discounts offered to members will not apply.

 

The Fine Print:

This is a dental discount plan and is not dental insurance.  It cannot be combined with any other insurance.  It is good only at the office of Galleria West Family Dental.  Therefore, if you are referred to a specialist, they will not offer this discount.  This plan is non-refundable.  No refunds are given if a patient chooses not to use their plan.  This plan is subject to change yearly.  If you choose to extend your payments by using Care Credit, the discount on non-included fees will be reduced from 20% to 10%, due to merchant fees.
